Please complete this form as as soon as possible before the Academic Essay Accelerator begins. Your responses will help me prepare for your student’s participation and understand their writing goals.

For students under 18, a parent or guardian should complete the form with the student. Students aged 18 or older may complete it themselves.

Please do not include sensitive medical information. If you would like to discuss a learning or accessibility arrangement, briefly note that request and I will follow up privately or you may contact me at jane.malone@polariswriting.com.
